LTC Social Worker- Elmwood Hills Healthcare

Elmwood Hills Healthcare in Blackwood is looking for a energetic and compassionate social worker to join our team working with dementia residents on our long term care floor.
Salaried Position
Starting salary at $60,000. Able to negotiate pending experience

Responsibilities for a social worker include:
  • Assists residents with emotional adjustment to facility.
  • As directed, interview new residents and records, social history, and assessments.
  • Completes the social services portion of the MDS, Care planning, baseline, etc.
  • Refers to psychological services as needed.
  • Schedules and attends and participates in interdisciplinary care plan meetings morning meetings, utilization review, etc.
  • Writes summaries and progress notes as directed.
  • Assists with discharge planning. Follows up after discharge.
  • Conducts required Cut Letters
  • Writes discharge summaries as assigned.
  • Develops a professional relationship with each resident and family.
  • Educates residents/ representatives on Advanced Directives/POLST.
  • Adheres to all Social Services Regulations.
Qualifications for a social worker:
  • Minimum Bachelor's Degree in Social Work from an accredited College.
  • Must obtain a CSW and/or LSW.
  • One to Three years of professional experience and/or training; or equivalent combination of education and experience.
